Filling out our secure online Patient Registration Forms are required to schedule an appointment.
*If you have filled out forms with us within the last three months, you do not need to fill them out again. Please contact us to schedule an appointment.
After you have completed the forms, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.
Once we receive these forms, we will call you to schedule.
*We can mail paperwork to you if you prefer or we offer paper copies located in the foyer of the building. There is a drop box as well for paperwork, referral slip & x-rays.
*NO paperwork, etc can be brought into the office on the day of your appointment.
Also, it is necessary to have your referral slip and x-rays emailed to us by your general dentist prior to your appointment.
