Dental Insurance

If you do not see your insurance provider listed above, please contact our office. We work with many insurance plans and will gladly verify your benefits and discuss your coverage before your visit.
Most dental insurance plans do not cover the full cost of treatment. Any estimated patient portion is due at the time of your appointment. As a courtesy, we will submit your insurance claim after your treatment is completed.
Please note: Insurance estimates are based on information provided by your insurance company and are not a guarantee of payment. Final reimbursement is determined by your insurance carrier once your claim has been processed.

Please remember that dental insurance is a contract between you and your insurance provider. Regardless of insurance coverage, patients are ultimately responsible for all fees associated with their treatment.

Appointment Cancellation Policy
To help us provide timely care for all patients, we require at least 24 hours’ notice for appointment cancellations or changes.
A $75 fee will be charged for missed appointments or cancellations made with less than 24 hours’ notice.
