INSURANCE: WE ONLY PARTICIPATE WITH UNITED CONCORDIA & DELTA DENTAL PPO/ PREMIER.

ALL OTHER INSURANCES ARE PAYMENT IN FULL UPFRONT. 

Most insurance plans do not cover 100% of the fee. You are responsible for paying that portion of the fee not covered by your insurance up front and on the same day as your appointment. 

We will call your insurance company the day before your appointment for an explanation of your root canal therapy reimbursement. You may contact your insurance company for an estimate prior to your appointment.

Once your treatment is completed, we will submit your claim to your insurance for you.

For your convenience we accept Visa, MasterCard, Discover and an already active CareCredit account. We do not accept American Express or checks, (personal or business). 

**Due to COVID, we only accept Credit Cards over the phone.

Please remember, you are fully responsible for all fees charged by this office regardless of your insurance coverage. Your coverage is a contract between you and your insurance company.

There is a $75 charge for ALL no-shows/ late cancels, (24 hour notice is required).
